1) The little pictures of the castle are all to indicate the major city/castle of the kingdom. Everything else is marked except these as it was getting too crowded with text. There aren't any landmarks missing its just that stuff like Fira Tower, Twilight Castle, etc. are all shown by that.
2) Black lines separate land from ocean, Blue lines show rivers, and red lines divide the kingdoms up. The one green line merely shows the way the trail goes in the Sky Kingdom.
3) Something of an obvious point, but I only included the basic stuff here. Aside from the landmarks there really isn't any more detail as it would look just too busy. So lots of places that have forests, or maybe mountains or caves, or other cities or whatever else you could see being in these places that aren't on the map. Doesn't mean they don't exist, just had to keep it simple enough to read.
